Routine Home Care

Whether our patients reside at home, in assisted living facilities, long-term care facilities, or personal care homes, Routine Home Care (RHC) is the most common care option for patients and families. This level of care allows patients to receive comfort and support in a familiar environment while maintaining access to physicians, nurses, home health aides, and chaplains within their own homes or living spaces.

Respite care provides short-term relief for caregivers, allowing the primary caregiver to take a break and recharge. Whether for a few hours or several days, respite care ensures that your loved one continues to receive appropriate care and support during this time.

General Inpatient Care (GIP)

General Inpatient (GIP) Hospice Care is one of the four levels of hospice care defined and regulated by Medicare. It represents the highest level of hospice care, designed to support patients who require short-term, acute symptom management that cannot be safely or effectively managed in their home or residential care setting.

 

GIP hospice care provides 24-hour skilled nursing care in a skilled nursing facility (SNF) that partners with a hospice agency. The Grove Care Centers of America has partnered with several facilities to ensure our patients receive the specialized care they need. Our goal is to stabilize the patient’s symptoms promptly, allowing them to safely return home and resume their previous plan of care once their condition improves

Crisis Care

Crisis care, also known as continuous care, is a specialized level of hospice service provided when a patient is experiencing an acute medical crisis that cannot be managed under routine home care. This short-term, around-the-clock nursing support helps manage acute symptoms and allows the patient to remain comfortable at home rather than being hospitalized.

 

Crisis care is typically initiated when a patient experiences uncontrolled symptoms such as:

• Severe pain

• Shortness of breath or respiratory distress

• Uncontrollable nausea or vomiting

• Agitation, including anxiety or restlessness

During crisis care, a nurse or licensed practical nurse provides extended bedside support, with physician oversight to adjust medications as needed. Emotional and spiritual support is also offered to both the patient and their family.

 

Once the patient’s symptoms are stabilized, care reverts to the routine hospice home care level. Crisis care is intended as a temporary intervention, usually lasting from a few hours to several days, until symptoms are under control.
